fcb7261625 Prevent valgrind false positive in rest_blockhash_by_height
A bad interaction between valgrind and clang 6.0.0-1ubuntu2 with -O2
optimizations makes valgrind misleadingly imply C++ code is reading an
uninitialized blockheight value in rest_blockhash_by_height just because that's
what clang optimized code is doing. The C++ code looks like:

During the rest_interface.py test:

   self.test_rest_request("/blockhashbyheight/", ret_type=RetType.OBJ, status=400)

when height_str is empty, ParseInt32 returns false and blockheight value is
never assigned. The optimized code reads the uninitialized blockheight value
in 0xc(%rsp) before the checking the ParseInt32 return value in %al, which is
harmless, but triggers the following error from valgrind:

This is a known bad interaction between clang and valgrind. The clang optimized
code is correct but valgrind has no way of knowing that accessing the
uninitialized value isn't a problem. Issue has been reported previously:

    https://github.com/Z3Prover/z3/issues/972

This commit just sets blockheight to 0 as a workaround.

This aliasing makes subsequent commits easier to review; eventually CoinsTip()
will return the CCoinsViewCache managed by CChainState.

631940aab2 scripted-diff: replace chainActive -> ::ChainActive()
Though at the moment ChainActive() simply references `g_chainstate.m_chain`,
doing this change now clears the way for multiple chainstate usage and allows
us to script the diff.

Pull request description:

  Motivated by https://github.com/bitcoin/bitcoin/pull/11913#discussion_r157798157, this pull makes `blockToJSON` and `blockheaderToJSON` free of `cs_main` locks.

  Locking `cs_main` was required to access `chainActive` in order to check if the block was in the chain and to retrieve the next block index.

  With the this approach, `CBlockIndex::GetAncestor()` is used in a way to check if the block belongs to the specified chain tip and, at the same time, get the next block index.

Consolidate redundant implementations of ParseHashStr
This change:
* adds a length check to ParseHashStr, appropriate given its use to populate
  a 256-bit number from a hex str.
* allows the caller to handle the failure, which allows for the more
  appropriate JSONRPCError on failure in prioritisetransaction rpc

30973e9844 [REST] improve performance for JSON calls
JSON calls do not use the raw data generated for
the .bin and .hex calls. By moving the raw data
creation into the .bin and .hex switch branches,
JSON calls become faster.

8ecaee13f7 Increase signal to noise in appveyor build output by reducing the MSVC warning count from 12 to 4 (12 is assuming the changes in #14086 are also implemented).
This makes it easier to spot errors or more important warnings in the verbose appveyor output.

6d772a3d44 [rpc] Public interfaces to GetTransaction block until synced.
Now that the transaction index is updated asynchronously, in order to
preserve the current behavior of public interfaces, the code blocks
until the transaction index is caught up with the current state of the
blockchain.

5083079688 Switch CCoinsView and chainstate db from per-txid to per-txout
This patch makes several related changes:
* Changes the CCoinsView virtual methods (GetCoins, HaveCoins, ...)
  to be COutPoint/Coin-based rather than txid/CCoins-based.
* Changes the chainstate db to a new incompatible format that is also
  COutPoint/Coin based.
* Implements reconstruction code for hash_serialized_2.
* Adapts the coins_tests unit tests (thanks to Russell Yanofsky).

A side effect of the new CCoinsView model is that we can no longer
use the (unreliable) test for transaction outputs in the UTXO set
to determine whether we already have a particular transaction.

d342424301 Remove/ignore tx version in utxo and undo
This makes the following changes:
* In undo data and the chainstate database, the transaction nVersion
  field is removed from the data structures, always written as 0, and
  ignored when reading.
* The definition of hash_serialized in gettxoutsetinfo is changed to no
  longer incude the nVersion field. It is renamed to hash_serialized_2
  to avoid confusion. The new definition also includes transaction
  height and coinbase information, as this information was missing
  before.

This depends on having a CHashVerifier-based undo data checksum
verifier.

Apart from changing the definition of serialized_hash, downgrading
after using this patch is supported, as no release ever used the value
of nVersion field in UTXO entries.

528472111b Get rid of nType and nVersion
Remove the nType and nVersion as parameters to all serialization methods
and functions. There is only one place where it's read and has an impact
(in CAddress), and even there it does not impact any of the recursively
invoked serializers.

Instead, the few places that need nType or nVersion are changed to read
it directly from the stream object, through GetType() and GetVersion()
methods which are added to all stream classes.

fb8a8cf2e6 rpc: Register calls where they are defined
Split out methods to every module, apart from 'help' and 'stop' which
are implemented in rpcserver.cpp itself.

- This makes it easier to add or remove RPC commands - no longer everything that includes
    rpcserver.h has to be rebuilt when there's a change there.
- Cleans up `rpc/server.h` by getting rid of the huge cluttered list of function definitions.
- Removes most of the bitcoin-specific code from rpcserver.cpp and .h.

Continues #7307 for the non-wallet.

9cdc3353c5 Simplify logic of REST request suffix parsing.
This patch changes the way the suffix (giving the requested data format) is
parsed for REST requests.  Before, the string was split at '.'
characters and it was assumed that the second part was the suffix.

Now, we look for the last dot and use that to determine the suffix.
This allows for strings that contain dots (not used now, though), and
seems, in general, to be clearer and more intuitive.

40b556d374 evhttpd implementation
- *Replace usage of boost::asio with [libevent2](http://libevent.org/)*.
boost::asio is not part of C++11, so unlike other boost there is no
forwards-compatibility reason to stick with it. Together with #4738 (convert
json_spirit to UniValue), this rids Bitcoin Core of the worst offenders with
regard to compile-time slowness.

- *Replace spit-and-duct-tape http server with evhttp*. Front-end http handling
is handled by libevent, a work queue (with configurable depth and parallelism)
is used to handle application requests.

- *Wrap HTTP request in C++ class*; this makes the application code mostly
HTTP-server-neutral

- *Refactor RPC to move all http-specific code to a separate file*.
Theoreticaly this can allow building without HTTP server but with another RPC
backend, e.g. Qt's debug console (currently not implemented) or future RPC
mechanisms people may want to use.

- *HTTP dispatch mechanism*; services (e.g., RPC, REST) register which URL
paths they want to handle.

By using a proven, high-performance asynchronous networking library (also used
by Tor) and HTTP server, problems such as #5674, #5655, #344 should be avoided.

What works? bitcoind, bitcoin-cli, bitcoin-qt. Unit tests and RPC/REST tests
pass. The aim for now is everything but SSL support.

Configuration options:

- `-rpcthreads`: repurposed as "number of  work handler threads". Still
defaults to 4.

- `-rpcworkqueue`: maximum depth of work queue. When this is reached, new
requests will return a 500 Internal Error.

- `-rpctimeout`: inactivity time, in seconds, after which to disconnect a
client.

- `-debug=http`: low-level http activity logging
