According to the BerkeleyDB docs, the DbEnv handle may not be accessed
after close() has been called. This change ensures that we create a new
handle after close() is called. This avoids a segfault when the first
connection attempt fails and then a second connection attempt tries to
call open() on the already closed DbEnv handle.
This change should make it easier for users to make complete backups of wallets
because they can now just back up the specified `-wallet=<path>` path directly,
instead of having to back up the specified path as well as the transaction log
directory (for incompletely flushed wallets).
Another advantage of this change is that if two wallets are located in the same
directory, they will now use their own BerkeleyDB environments instead using a
shared environment. Using a shared environment makes it difficult to manage and
back up wallets separately because transaction log files will contain a mix of
data from all wallets in the environment.
Remove requirement that two wallet files can only be opened at the same time if
they are contained in the same directory.
This change mostly consists of updates to function signatures (updating
functions to take fs::path arguments, instead of combinations of strings,
fs::path, and CDBEnv / CWalletDBWrapper arguments).

Make sure wallet databases have unique fileids. If they don't, throw an error.
BDB caches do not work properly when more than one open database has the same
fileid, because values written to one database may show up in reads to other
databases.
Bitcoin will never create different databases with the same fileid, but users
can create them by manually copying database files.

Now using a std::unique_ptr, the Db instance is correctly released
when CDB initialization fails.
The internal CDB state and mapFileUseCount are only mutated when

Instead, CWalletDB() with a dummy handle will just give you a no-op
database in which writes always succeeds and reads always fail. CDB
already had functionality for this, so just use that.
Abstract database handle from explicit strFilename into
CWalletDBWrapper.
Also move CWallet::Backup to db.cpp - as it deals with representation
details this is a database specific operation.
This changes the logging categories to boolean flags instead of strings.
This simplifies the acceptance testing by avoiding accessing a scoped
static thread local pointer to a thread local set of strings. It
eliminates the only use of boost::thread_specific_ptr outside of
lockorder debugging.
This change allows log entries to be directed to multiple categories
and makes it easy to change the logging flags at runtime (e.g. via
an RPC, though that isn't done by this commit.)
It also eliminates the fDebug global.
Configuration of unknown logging categories now produces a warning.

BerkeleyDB dump files have key and value lines indented.
The salvage code passes these to ParseHex as-is.
Check this in the tests (should just pass with current code).
Check for EOF before every getline, and warn when reading gets to EOF
before the end of the data.
Stricter error checking could shed more light on issues such as #7463
and #7379.
